What it Costs
UHC High Deductible Health Plan with Health Savings Account (HSA): $1,500 individual deductible; $3,000 per family. You must meet the deductible before the plan shares in costs. ServiceNow will contribute to your HSA, up to $1,000 per year for single coverage and up to $2,000 per year for family coverage. The amount you receive is prorated based upon your date of hire.
UHC PPO: $500 individual deductible; $1,500 per family. $25 copay for primary physician and specialist visits.
Kaiser Permanente HMO: Available in California only. No annual deductible and a small copay for most services.
In addition to your health plan, you’re also covered for mental health, counseling, and work/life services through Lyra Health.
Dental and Vision
Delta Dental: Annual benefit of $2,000; Cleanings covered three times per year and bitewing x-rays
twice per calendar year. Orthodontia is also covered up to a lifetime maximum of $1,500.
Vision Service Plan: Exam and lenses every year; frames every other year. $10 exam fee and $25 for materials. Lenses covered in full after materials copay. Frames covered up to $120 in-network ($70 at Costco).
Health Savings Account (HSA): If you choose the UHC High Deductible Health Plan, you qualify for an HSA. This lets you save money pre-tax to pay for medical expenses. And, yes, you get to keep your account even after your internship. We’ll even start you off on the right foot: we contribute up to $1,000 into your HSA for single coverage and up to $2,000 for family coverage. The amount you receive is prorated based upon your date of hire.
401(k) Plan: You can open up a 401(k) account to save pre-tax dollars for retirement. It’s a long way’s away but, once you get there, you’ll be glad you started early.
As a high tech company, we know we have a high bar to hit as far as office culture goes. Here are just some of the on-site perks you’ll be able to take advantage of (vary by location):
